Kayleigh is a mother of three, including Pippa Griffiths, who died avoidably at Shrewsbury and Telford Hospital NHS Trust (SaTH), aged one day old.

After being concerned about what other events were being hidden at SaTH, Kayleigh joined forces with another bereaved mother, Rhiannon, and wrote to the then Secretary of State for Health and Social Care, Jeremy Hunt, who agreed to commission the Ockenden Review.

Jane O’Hara is Professor of Healthcare Quality and Safety, based within the School of Healthcare, University of Leeds, UK. She is Deputy Director of the Yorkshire Quality and Safety Research Group, and theme lead for the Patient Involvement in Patient Safety theme within the NIHR Yorkshire & Humber Patient Safety Translational Research Centre. Jane also holds a Visiting Professor position at the SHARE Centre for Resilience in Healthcare at the University of Stavanger, Norway.

Jane has over a decade of experience leading patient safety research, and a further eight years of applied psychological research prior to that. Currently, Jane leads two large grants funded by the NIHR Health Services & Delivery Research programme, and is co-applicant on grants funded by THIS Institute, the Trondheim Foundation and Research Council of Norway, and the NIHR Programme Grants for Applied Health Research.

As a registered mental health nurse of over 20 years, Steve has held a wide variety of clinical, leadership and managerial positions within NHS trusts, as well as fulfilling the role of a specialist advisor for the Care Quality Commission. During this time, Steve has worked on developing clinical policy, service improvement initiatives, and staff development. He has also developed and delivered clinical risk training within mental health services.

Steve is a trained and experienced state level healthcare safety investigator, having worked as maternity investigator for the Healthcare Safety Investigation Branch. He has also worked with Mental Health Trusts completing investigations in response to patient safety incidents and coached others through the investigation process and the production of high-quality safety reports.

Steve is currently working towards technical membership of the Chartered Institute of Ergonomics and Human Factors.
